Synthesizing Lessons from IMPaCT and Transforming Primary Care Initiatives


During the past four years, the Agency for Healthcare Research and Quality (AHRQ) has invested more than $12 million to 33 recipients in three separate grant initiatives to better understand the challenges of primary care transformation and to help create an infrastructure to assist primary care practices in their efforts to transform into patient centered medical homes.

These investments include:
  • Transforming Primary Care Practice (TPC) grants to study the process of transformation, determine metrics to evaluate the success of transformation efforts, and share those lessons with the field;
  • Infrastructure for Maintaining Primary Care Transformation (IMPaCT) grants for state level initiatives to create collaboratives that model and disseminate their successful initiative with three to four additional state programs; and
  • Estimating the Costs of Supporting Primary Care Practice Transformation grants to provide information about the costs of implementing and sustaining transformative primary care practice redesign.
Abt Global – in partnership with subcontractors the MacColl Center for Health Care Innovation at Group Health Research Institute and Corrigan/Kantz Consulting – leads the development of materials to synthesize and disseminate findings across these three grant initiatives. This includes developing:
  • Project profiles for all 33 grants;
  • An annotated bibliography of every journal article and report produced by the TPC grantees;
  • A catalog of tools, resources, publications and other information developed by each of the 4 IMPaCT grantees and the 13 affiliated state programs on "how to" transform primary care;
  • Success stories for each IMPaCT project;
  • A practical guide to measuring the costs of primary care based on the methods used and initial results from the Estimating Costs of Supporting Primary Care Practice Transformation; and
  • Content for AHRQ's website to highlight these initiatives and new products.
